Our Order Form has Changed!! Please read!!

New Ohio sales tax laws require that we collect sales taxes for our mail orders based on the county of the RECIPIENT, rather than our county (thank you, Governor and Legislators) making a potential nightmare of record keeping.

We thought about discontinuing our mail order business completely, and having you all travel here to our school to make purchases (after all, if senior citizens go to Canada for drugs...) Anyway, we decided that we would first try a different approach.

With our new order form, you fill it all in while you're online. You fill in such obvious fields as name, address, phone, Email address, school, and your role at your school.

*** Then, and this is critical, you must correctly select YOUR COUNTY from the pull down list on the form. Please be careful to select correctly. IF YOU DON'T LIVE IN OHIO, THE BOTTOM OF THE LIST HAS THE "NOT OHIO"
CHOICE.***

Then, in the quantity fields for each item, indicate the number of each item that you wish to purchase.

FINALLY, press the button near the buttom of the form that says "Total and Print"... the computer should automatically fill in the costs of the items, the sales tax, the shipping charges, and the grand total, and will send a computer toward your printer. Take that printed copy - sign and date it at the bottom - and mail it to us with your payment.

Please note: you MUST be on a Javascript-compliant Web Browser (such as Microsoft Internet Explorer, or Netscape's Browser) for this to work. And, we have no way to check if this works on a MacIntosh computer's operating system, so if it fails, we can only suggest that you please try again from someone else's computer that uses Microsoft Windows.
